JOB DESCRIPTION:

SUMMARY OF POSITION

Facilitate community outreach and disseminate educational information to participants, organizations, & stakeholders with the goal of increasing understanding of addiction, recovery, medicated assisted treatment, and full continuum of care offered by Centerstone. Increase understanding of medical necessity and how to make a referral to Centerstone's RCORP Impact grant program encompassing linkage to outpatient, residential and recovery housing, community resources and other providers as needed. Provide assistance in 7 rural counties Brown, Jackson, Jefferson, Jennings, Lawrence, Owen, and Scott counties.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Work within Centerstone SUD Residential, Clinic, and Grant service lines to provide education and connect people to appropriate services. Foster ongoing communication with all of these large systems within Centerstone.
  • Obtain referrals/enrollments for the RCORP Impact grant.
  • Interact with individuals seeking services in rural counties, complete enrollments, and assist with transportation to care as needed, and updating referral sources.
  • Coordination with service providers to increase collaboration and increase in referrals.
  • Maintain effective working relationships with the clinical team and community partners. Track all community interactions/ dissemination of information.
  • Assist with client follow up

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Travel and work in all 7 counties as needed.
  • Understanding of building community stakeholder groups, implementation of new initiatives is ideal.
  • Understanding of addiction, recovery, medicated assisted treatment, medical necessity for level of care is a must.
  • Must be willing to work with clients individually as well as interacting with community/ referral sources.

QUALIFICATIONS

Education Level

Bachelor's degree in social work, psychology, or related field is required.

Years of Experience

Three to five years' experience in behavioral health environment is desired. Experience with facilitation and community outreach is a must.
