Responsible for securing patient data based upon comprehensive data elements, financial education/counseling, insurance verification, provision of clinical information for pre-certification, authorization and third party claims management. Interviews patient for clinical, financial, and history information and arranges appropriate clinical care. A Clinical Pharmacy Technician will work independently gathering and submitted patient information with oversight of a pharmacist as needed. They will be responsible for performing pharmacy related functions in compliance with departmental policies and procedures that provide optimal pharmaceutical care.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. High school diploma or Equivalent.

2. Five (5) year's pharmacy technician or healthcare experience AND nationally certified (PTCB or EXCPT) within 15 months of hire AND Registered with the applicable state Board of Pharmacy within 15 months of hire (except PA).

3. Must complete job specific competencies or obtain specialty certification within 90 days of hire.

4. Must be registered with the state Board of Pharmacy as a Pharmacy Technician trainee upon hire (except PA).

PREFERREDQUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Associates degree OR Post high school education in a science field of study OR graduate of a pharmacy technician training program.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Drafts various correspondence letters and sends to referral source when necessary.

2. Gathers and documents patient demographics, insurance information, needs assessment, patient history, and other data as needed.

3. Initiates refill/follow up calls and schedules delivery as needed.

4. Provides information and answers to patient/caregiver questions and refers to clinician when appropriate or requested.

5. Identifies all patients requiring pre-certification or pre-authorization at the time services are requested or when notified by another hospital or clinic department.

6. Contacts insurance company or employer to determine eligibility and benefits for requested services.

7. Maintains patient records in a timely and complete manner.

8. Assists the pharmacist with the process of filling and labeling the prescriptions.

9. Calculates initial measurements for compounds and maintains compounds logs.

10. Provides monographs and other required medication guides with each prescription.

11. Maintains proper storage of medications (i.e. security, temperature controlled items, etc.).

12. Demonstrates the knowledge and skills necessary to communicate to the infant, pediatric, adolescent, adult and/or geriatric patient per the established age-specific education/standard.

13. Works with coordinators and manager to develop and attain quarterly objectives.

14. Maintains confidentiality according to policy when interacting with patients, physicians, families, co-workers and the public regarding demographic/clinical/financial information.

15. Communicates problems hindering workflow to management in a timely manner.

16. Initiates charge anticipation calculations. Accurately identifies anticipated charges to assure identification of anticipated self-pay portions.

17. Participates in educational programs to meet mandatory requirements and identified needs with regard to job and personal growth.

18. Attends departmental meetings and/ or documents review of meeting minutes.

19. Participates in quality activities as assigned by Pharmacist Manager.

20. Participates in performance improvement (i.e. follows established work systems, identifies deviations or deficiencies in standards/systems/processes and communicates problems to coordinator or manager)

SKILLS AND ABILITIES:

1. Must be able to use computers and software applications.

2. Knowledge of drugs, drugs dosing, side effects, indications, contraindications and proper administration preferred.

3. Knowledge of rules, policies and regulations related to the disbursal of pharmaceutical drugs preferred.

4. Must be able to work accurately, efficiently, and safely in a fast-paced environment.

5. Must be polite and respectful when communicating with staff, physicians, patients and families. Must approach interpersonal relations in a positive manner.

6. Must demonstrate flexibility to meet the needs of department or hospital in regard to changes in work volume, planned change, scheduling changes and emergency call-backs (i.e. weather or disaster).

7. Must be able to communicate effectively verbally and in writing and place high emphasis on customer service.

Additional Job Description:

Diversion Specialist:

* Works directly with Control Check software to identify any potential diversion issues with delivery, dispensing, administration and waste of controlled substances.

* Communicate with physicians and nurses to reconcile discrepancies within the Control Check system.

* Review IRIS scores for nursing staff and alert pharmacy administration for any potential concerns.

* Reconcile controlled substances for IV infusion administration to account for drug administered and waste on discontinued medication.

* Review daily reports such as discrepancy report, exception report and override report.

* Communicate with nursing and pharmacy staff to ensure reconciliation of cabinet discrepancies and restock exceptions in a timely fashion.

* Reconcile and validate high dollar queue.

* Review and complete document waste for billing report.

* Must work closely with pharmacy administration and communicate findings and as they arise related to billing or diversion issues.

* Schedule will be Monday - Friday 8AM-430PM
