Responsibilities

Summary
The Assistant Case Manager assists operations of the facility by answering and responding to phone calls, filing, data entry, completing Urinalysis screens on participants, coordinating with client's Probation Department regarding new participants, as well as established participant behavior. It is also responsible for completing Starting Point with new participants and completing the data entry. Assistant Case Managers will also, when necessary, facilitate cognitive based skill building groups, as well as carry a small case load to assist case managers. Assistant Case Managers that carry a case load will be responsible for all case management aspects of those participants including, conducting one on one individual cognitive based interventions, referrals to outside resources, collaboration with probation officers regarding coordination of care. Assistant Case Managers will also complete all other duties as assigned by Program Manager.

Primary Duties and Responsibilities

  • The Assistant Case Manager checks-in all participants entering the facility. May complete drug screens according to assigned random schedule for each participant check-in.
  • May perform breath analysis on each participant who enters the facility for any purpose and provides a written or computerized result.
  • The Assistant Case Manager completes initial intake process with participants to completion, including intake paperwork, input into Company database, and assignment of groups and other services.
  • Provides general office administration duties, including answering phones. Provides general customer service to all who enter the facility, including participants, customers and the general public.
  • Assists case managers with case coordination, communication with appropriate official agencies, and preparation and distribution of reports.
  • Files, prepares charts and performs data entry as assigned.
  • May be responsible for office opening and closing procedures.
  • May facilitate orientation and psycho-educational/behavior modification groups on a weekly basis. Documents participants' attendance, participation and progress in Company database.
  • May hold a small case load of participants and oversee case management functions of those assigned.
  • Performs other duties as assigned.

Qualifications

Minimum Requirements

  • High school diploma required.
  • Two (2) years college coursework in Human Services or business discipline preferred.
  • Minimum of six (6) months experience in Human Services field with direct participant services preferred.
  • Minimum of six (6) months experience working with minority populations preferred.
  • Must be twenty-one (21) years of age with at least five (5) years driving experience if required to transport participants.
  • Good customer service skills.
  • Effective communication skills with internal and external contacts at all levels.
  • Good organizational skills and attention to detail.
  • Solid computer skills and proficiency with MS Word and Excel.
  • Basic database skills.
  • Basic math skills.
  • Ability to work with computers and the necessary software typically used by the department.
