RN opportunity with Cardiovascular Institute (CVI) on the HeartNet Team. Seeking self-motivated individual to join our team. HeartNet team key responsibilities include collaboration between our HeartNet hospital referral network as well as Emergency Medical Services across our region and state. HeartNet facilitates education and outreach for our hospital and community partners. Primarily day shift schedule with some weekends. Hybrid remote opportunity. As part of the healthcare team, coordinates resources and utilizes nursing assessment to facilitate patient transfer and admission from referring physicians, HeartNet and other hospitals/facilities, primarily from Southwest Virginia and West Virginia.
- Facilitates the safe and timely transfer of patients from referral sources to CRMH. Utilizes clinical knowledge, skills and experience to facilitate patient placement and communication of specific patient information to receiving providers. Serves as liaison between referring provider/facility and CRMH team. Updates team with pertinent information and/or changes in patient condition.
- Maintains positive relationships and communication with entire healthcare team and all stakeholders. Serves as patient/family advocate for specific needs while at CRMH. Provides timely, confidential feedback to referring facility.
- Participates in the development, implementation and evaluation of programs that address educational needs of the community and staff at all HeartNet hospitals. Participates in planning of annual Cardiac/Heartnet Symposium.
- Participates in clinical, leadership and educational opportunities that support evidence-based, quality patient care and monitoring of patient outcomes. Collects and analyzes data, makes recommendations for improvement and collaboratively assists with implementation and evaluation of strategies aimed at accomplishing organizational/departmental goals.
- Functions as a role model for clinical care for healthcare team members and students. Offers to assist in providing educational experiences for nursing and other disciplines.
- Supports cardiology research initiatives and performance improvement activities.
What We Require:
Education: Graduate of state approved School of Nursing.
Experience: 3 years of RN experience required. Cardiology experience preferred.
Licensure/Certifications/Registration: Current RN license by Department of Health Regulatory Boards of Commonwealth of Virginia; certification in cardiovascular nursing specialty preferred.
Other Minimum Qualifications: Excellent verbal and written communication skills. Ability to prioritize and organize. Flexible hours, with ability to travel overnight. Ability to use computer for communication, word processing, and maintenance of records and files.
