Summary
The Hospice RN Case Manager provides skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. The Hospice RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.
This position is part of the Massachusetts Nurses Association Bargaining Unit at Cooley Dickinson VNA & Hospice.
This position reports to the Hospice Manager and operates within established organizational and departmental policies and procedures.
Must be able to effectively interact appropriately to an individual's intellectual level and physical ability: pediatric, adult, geriatric.
Essential Functions-Make assigned initial/admission visit to prospective client's home to:
*Explain policy, services and limitations to clients/caregivers;
*Assess needs of client/caregivers to established appropriateness of care;
*Obtain informed consent signatures of client/caregivers and physician; and
*Explain client's rights.
-Develop a nursing assessment through collection of client/caregiver data that includes:
*Client's physical, emotional, spiritual, social, psychological and cultural status;
*Coping strategies, support system, and learning needs of client/caregivers;
*Safety in a hospice setting; and
*Reassessment in a systematic and ongoing manner.
Analyze the assessment data in determining diagnoses:
*Identify problems that may be resolved, diminished or prevented through nursing and/or interdisciplinary intervention;
*Validate diagnoses; and
*Form the initial outline for the interdisciplinary ID care plan in light of the diagnoses.
-Identify expected outcomes individualized to client/caregivers.
-Implement ID Care Plan that:
*Provides direct skilled nursing care and emotional support;
*Maintains accountability for all aspects of client's care outcomes, including supervision of HHA's;
*Communicates to other ID team members so care reflects hospice's interdisciplinary practice;
*Updates care plan as needed;
*Assures interventions are consistent with established plan of care;
*Ensures availability and accessibility of nursing care twenty-four (24) hours a day, seven (7) days a week; and
*Interventions are documented in a timely manner.
-Evaluate client/caregiver's progress towards attainment of outcomes.
-Maintain established agency, hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection control and environmental standards.
-Maintain professional growth and development through attendance at seminars, workshops, conferences or in-services, professional affiliations or journals to stay abreast of current trends in field of expertise.
-Meet annual competency and retraining requirements. Maintain any certifications required for the Hospice Nurse Case Manager position.
-Attend meetings as required.
-Perform other functions/duties as requested.

Education
Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred

Licenses and Credentials
Class D Passenger Vehicle Driver's License [State License] quired Registered Nurse [RN - State License] -

Experience
Minimum of one (1) year medical/surgical nursing experience required Hospice/home care experience preferred

Knowledge, Skills and Abilities
- Graduate of an approved, accredited school of professional nursing required.
- Demonstrated excellent verbal and written communication skills required.
- Demonstrated strong interpersonal, organizational, and time management skills required.
- Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required.
- Good driving record, and reliable transportation required.

Provide end of life care for patients and their families; Monday through Friday 4:30 pm-9pm with on call from 9pm-8am.
