PHSO iCMP PLUS Primary Care Physician

Examines and treats patients receiving clinical services for complex care to ensure proper injury care, disease prevention, diagnosis, treatment and recovery. Specialist knowledge of geriatrics, disability, behavioral health, and addiction medicine may be required.

  • Cares for a panel of complex primary care patients with high medical and/or behavioral health complexity enrolled in the iCMP PLUS program
  • Completes face-to-face visits in the home or community and non-face-to-face work associated with caring for a panel of patients.
  • Examines and treats patients to ensure proper injury care, disease prevention, diagnosis, treatment, and recovery. Professionally responsible for decisions with respect to diagnosis and treatment of patients.
  • Prescribes medications, performs routine vaccinations, and provides advice regarding personal health and hygiene.
  • Requests necessary tests, follow-up visits and refers patients to specialists, as necessary. Follows up results from any ordered tests and communicates these to patients in a timely fashion.
  • Completes continuity visits with patients in the Emergency Department, inpatient hospital, and/or MGB office setting as needed.
  • Documents all patient encounters (in person, virtual, or phone) in an accurate and timely manner in accordance with MGB and PHSO policies.
  • Accurately codes patient conditions (HCC coding) and actively addresses quality gaps for chronic conditions.
  • Available to respond to patient concerns during business hours (M-F 7:45 am - 5:15 pm per PHSO program policies. After hour and weekend coverage will be provided by Mass General Brigham Medical Group.
  • Collaborates with multi-disciplinary team to holistically care for patient needs
  • Relies on experience and judgment to plan and accomplish goal

  • Adhere to MGB iCMP PLUS Policy & Procedures
  • Promptly, in good faith, report any instances of suspected fraud, waste and abuse; suspected privacy and/or security incidents; or any compliance concerns identified
  • Ensure confidentiality of patient and company proprietary information is maintained

Licensure and Qualifications

  • Doctor of Medicine degree (MD)
  • Residency training in internal medicine or family medicine
  • Valid and unrestricted Massachusetts license to practice the profession
  • Valid DEA license for prescribing of controlled substances
  • 3-5 years experience working as a primary care physician
  • Familiar with standard concepts, practices, and procedures within a the field of outpatient adult medicine. Relies on experience and judgment to plan and accomplish goals.
  • Familiarity and proficiency in diagnosis, treatment, and prescribing for primary care behavioral health conditions (e.g. depression, anxiety, ADHD) and substance use disorders in the outpatient setting.
  • Familiarity and proficiency with the specific medical, social, and community needs of people with disabilities or chronic serious illness and elders
  • 'Experience in patient engagement, serious illness and goals of care conversations, motivational interviewing, and team-based care.
