Responsibilities
Due to continued growth, we are seeking an experienced weekend PRN Social Services Case Manager with recent behavioral health experience. Essential Job Functions include but are not limited to providing clinical social work expertise to hospital wide programs; deliver social services in assigned areas according to patient needs; adhere to standards of patient quality care and work as a member of the interdisciplinary treatment team within guidelines set by the Social Services Department and the individual unit Program Director. SummitRidge Hospital -is a place for Healing and Hope! Our behavioral health psychiatric hospital is in Lawrenceville (about 25 miles northeast of downtown Atlanta). We provide psychiatric and substance use disorder treatment to adolescents, adults, and seniors, in addition to partial hospitalization and other outpatient programs (includes a location in Athens). Current professional licensure is strongly preferred. Recent experience in individual, group, and family therapy. Ability to assess the needs and provide the required range of treatment modalities and interventions. Preferred: a minimum of one year experience in discharge planning, clinical case management or a similar position in a community based agency setting which provides experience in referrals for patients.
