Clinical Pharmacist - MGH Primary Care

As a member of the primary care team, the Ambulatory Care Clinical Pharmacist will be responsible for the provision of safe and effective patient centered pharmacotherapy in the primary care space. Clinicians are aligned with targeted interdisciplinary programs and specialty services to manage chronic disease states through Collaborative Drug Therapy Management agreements (CDTM).
The following are essential job functions for the Ambulatory Primary Care Clinical Pharmacist:
*Leads assigned patient service lines, clinical areas, and therapeutic
programs
*Delivers direct patient-care and clinical practice, including decentralized
and service-based programs
*Provide services to ensure optimal transitions of care, including collecting,
reconciling, and correcting patient medication discrepancies
*Work collaboratively with the patient's provider to establish patient centered
disease state goals
*Create and implement patient centered therapy plans as outlined by current
literature and scope specific agreements, policies, and guidelines
*Implements, evaluates, and redesigns pharmaceutical care plans based on
patient progress, evolving goals for medication usage, adverse drug
reactions, and adherence. This includes adjustment of medications,
ordering labs for monitoring, and titration of medications as outlined in the
policies and procedures
*Accurately orders medications, vital signs, or pertinent laboratory tests for
patients
*Provides written documentation in the patient's medical record for all
patient encounters
*Educates patients regarding safe and effective use of medications,
possible medication side effects, and the role of prescribed medications in
the overall therapeutic care plan
*Facilitates experiential education and practice advancement
*Maintains proficiency in the four preceptor and actively emulates
education/mentorship skills
*Serves as a preceptor/educator for MGH pharmacy residency (PGY1 and
PGY2) and student experiential education programs, including maintaining
training and experience in pharmacy practice areas appropriate for
teaching assignments

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:

Current licensure as a pharmacist in the Commonwealth of Massachusetts (or eligible) required Board Certification in area of expertise (BCACP, CDE) is required within 18 months of hire (if not already certified)

EDUCATION:

Graduate of an ACPE accredited College of Pharmacy required. Pharm.D. or other advanced degree preferred Completes required additional continuing education requirements including completion of at least one hour of pain training every 2 years and additional 5 contact hours of CE in the area of credentialing yearly. Maintains active credentialing status as well as MA controlled substance license and completes all required documentation in a timely manner.

EXPERIENCE:

Completion of ASHP accredited PGY1 Pharmacy Residencyrequired, and PGY-2 Ambulatory Care residency preferred, or equivalent experience Clinical pharmacy experience (1-5 years) in ambulatory care settings and/or CDTM experience preferred; required if no PGY2 residency training Exceptional aptitude or knowledge may be considered in lieu of experience. Extensive clinical experience and development may be considered in lieu of formal training
