Community Nurse

Yamhill County has one regular full-time position for a Licensed Practical Nurse in our ACT/EASA program. This position will involve work with evidence-based models including Early Assessment and Support Alliance (EASA)http://easacommunity.org/ and Assertive Community Treatment (ACT) https://oceact.org/. The individual in this position may provide both general and specialized nursing services for individuals with serious mental illness emphasizing medication administration, monitoring, medication skills training, medical case management, illness prevention, working with pharmacies, setting up accounts for free medication services, giving injections, and tracking, documenting, and coordinating with physicians as needed. Duties and responsibilities of this position will be carried out in both the clinic and in the community.

The Qualifications:

• Currently licensed to practice as a Licensed Practical Nurse in the State of Oregon
  • Two years' experience as an LPN preferred.
• New Graduates are welcome to apply!
• A National Provider Number (NPI) or be able to obtain one upon employment.
• A valid license to drive in Oregon and a good driving record.
• Any satisfactory combination of experience and training which ensures the ability to perform the work may substitute for the above.

Preference may be given to candidates with a bachelor's degree, public health and/or behavioral health experience, and/or Spanish language skills.

Will be subject to successful completion of a background check.

The successful candidate must possess excellent communication skills, be organized and detail-oriented, and possess the ability to work effectively alone or in a team environment, interacting with co-workers and the public in a courteous, professional manner. The ability to work with a multi-cultural clientele is required. Must have excellent customer service skills and be proficient in computer use including knowledge and experience with Microsoft Word, Outlook, and Excel.
