Hospital Care Mgmt Job Summary

Under direct supervision, responsible for 1) ordering, distributing, stocking, and maintaining the appropriate amount of inventory of supplies and medical equipment to support the inpatient nursing departments, the outpatient clinics, and other clinical and support departments; 2) maintaining a relationship with various DME vendors; and 3) processing, tracking and reporting of consignment closet equipment handled through the Hospital Care Managment Department.

Essential Functions:
  • Maintain accurate records of all Durable Medical Equipment that is provided via indigent Care Funds (ICF). (5%)
  • Be able to guide providers on qualifying language, that will adjudicate medical necessity to ensure insurance coverage. (15%)
  • DME Face-to-Face note: F2F notes are a vital part of the justification process and documentation provided. (5%)
  • Conduct follow-up phone calls with DME providers as needed, to confirm referral receipt and obtain disposition. (5%)
  • Manage ordering and procuring DME/O2 from vendors while maintaining equipment to ensure consistent on-demand availability for both inpatient and outpatient services (DECC, AUCC, CDU and Emergency Department). (25%)
  • work closely with hospital staff to assess the Durable Medical Equipment needs of patients to continue the continuity of care. Collaborates with Social Workers, RN Care Coordinators and other clinical staff in determining medical necessity for requested equipment. (20%)
  • Work with DME platform (Parachute Health) to streamline E-prescribed orders reducing paper scripts. (5%)
  • Work with DME Company/ Clinical Staff to enhance overall patient clinical outcomes to reduce Hospital Re-admission (5%)
  • Ensure that all equipment purchased a/o provided meets regulatory requirements and hospital policies. (5%)
  • Knowledgeable with all Medicare regulations and compliant with Medicaid regulations in Colorado (10%)

Education:
  • High School Diploma or GED Required

Work Experience:
  • 1-3 years Prefer at least 3 years experience working in a hospital setting, preferably in a role related to Durable Medical Equipment.

Licenses:

    Knowledge, Skills and Abilities:

    • Extensive knowledge of insurance coverage/policies and ICD-10.
    • General understanding of Medical Terminology.
    • Excellent verbal and written communication skills, including ability to communicate effectively with staff and patients.
    • Detail oriented, ensure accurate documentation and care is implemented for all patients.
    • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary patient care.
    • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines and standard accepted practices.
    • Receive and process incoming DME orders in accordance to patients' insurance while providing patient choice.
