Compensation Grade: P18Compensation Details: Minimum: $63,887.00 - Maximum: $63,887.00 AnnuallyDepartment (OPH) CCH - Division of Chronic Disease PreventionJob Description: Responsibilities The Health Program Coordinator will work in the New York State Department of Health Bureau of Community Chronic Disease Services, Disability and Health Program (DHP). The overarching goal of the DHP is to advance access to healthcare and improve health outcomes for people with disabilities. The incumbent will contribute to developing health promotion materials designed for people across different types of disabilities, with focus on preventative care, chronic disease management, and self-advocacy related to healthcare. The incumbent will also contribute to developing content for trainings for practitioners and service providers to improve communication and overall engagement with people with disabilities in healthcare and public health, as well as improving understanding of barriers created by lack of physical and programmatic accessibility. The Health Program Coordinator will support information gathering activities through participation in planning meetings, development of survey and focus group questions, dissemination of surveys, coordination of focus groups, note-taking, and assisting in reviewing feedback from focus group and survey participants. Minimum Qualifications Bachelor's degree in a related field and two years of experience contributing to the coordination of program activities in a public health, health, health regulatory, or human services related program; OR an Associate's degree in a related field and four years of such experience; OR six years of such experience. A Master's degree in a related field may substitute for one year of experience. Preferred Qualifications
Conditions of Employment Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required. Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position. The selected candidate must reside within a reasonable commuting distance of the official work location specified in the job posting and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire.
