JOB DESCRIPTION:

Supervises Family Support Program (FSP) and coordinates targeted case management for children and youth. Helps coordinate Family Support Program (FSP) and Specialized Family Support Program (SFSP) covered services. Assists staff with case management, access to community mental health services, community support, alternative community services, and residential treatment when it is determined to be medically necessary. CCBHC

* Provides supervision and consolation to FSP specialists, other case management and community support providers and interns/practicum students, as assigned to assure quality of services. Providing back up for FSP specialists support and back up on their cases during PTO and other times they are unavailable.

This also includes consistently returning phone calls and e-mails within 24 hours and updating voicemail/out of office e-mail, as well as consistently using pleasant tone and having a helpful attitude.

* Coordinate targeted case management staffings and facilitate seemless transition to other appropriate programs.
* Provides education and communication about FSP and targeted case management services to internal and external providers.
* Completes enrollment for clients entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, conducts assessment and writes assessment summaries, making discharge plans and provide follow up as assigned.
* Assists clients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
* Identifies and assists with Therapeutic Support Services and Individual Support Services, including completion of required prior authorization from HFS and providing necessary documentation to Accounts Receiveable department as needed.
* Participate in recruitment and hiring, and maintain primary responsibility for training, supervision, disciplinary action, and performance appraisal of assigned staff and interns. Work cooperatively with Coordinator to ensure that clinical supervision is consistently provided and is of the highest quality.
- Effectively communicate via written, verbal, in person and virtual methods.
- Knowledge of guidelines for confidentiality

- Knowledge of applicable regulatory guidelines and codes, agency policies and procedures, and Federal and state laws pertaining to position

- Skill in establishing and maintaining interpersonal relationships with clients and staff

- Skill in utilizing computer databases, such as electronic health records, eQHealth or other such programs.
- Knowledge of community geographical awareness and of community and agency resources.
- Skill in customer service techniques.
- Skill in developing problem solving strategies.
- Skilled in consultation and networking.

Pay Range:

$55,900.00--$78,330.00
