Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. We hire people that are skilled and passionate to meet our mission to end homelessness through outcome-based strategies which support personal and community transformation.

Flip the Script (FTS) is a voluntary, African American, culturally specific program that supports men/women who are justice involved. The FTS case manager acts to ensure the stability of all participants, is familiar with and able to access resources for benefits/entitlements, alcohol & drug treatment, mental health treatment, peer support, employment support, housing support & advocacy support.

Schedule: Monday - Friday, 8am - 4:30pm

Location: 305 SW 4th Ave. Portland, OR 97204 (on 4th and Oak)

Compensation: Starting at $22.19/hr

MINIMUM QUALIFICATIONS:



  • High school diploma or GED and six months of experience working with the homeless population, behavioral health environment, human services, or related environment; OR 6 months experience in a high-volume customer service role.
  • Experience working with justice involved individuals.
  • Experience working with homeless and/or recovering chemically dependent individuals strongly preferred.
  • Must not actively be on parole or probation.
  • Must have sufficient computer skills to accomplish duties.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Must pass a pre-employment drug screen and background check.
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • Must adhere to agency's non-discrimination policies.

DESCRIPTION OF DUTIES:



  • Provide case management services for special needs offenders referred by the Department of Community Justice
  • Stay in communication with clients, parole/probation officers and treatment providers.
  • Complete intake/interview process of clients upon referral
  • Assist clients in collaboration with P.O.'s and housing providers to transition to housing.
  • Facilitate groups.
  • Attend weekly staff meetings
  • Develop and maintain working relationships with treatment providers and other outside resources for clients.
  • Go inside the jails & prisons to share information about the FTS program
  • Participate in culturally specific outreach efforts to engage prospective clients, including appointments, home visits, and community-based services.
  • Develop relationships with organizations, providers, community leaders, and advocacy groups serving the African American community.
  • Use culturally sensitive lens to guide all aspects of service delivery, ensuring that the impact of socio-political & cultural factors is consistently considered
  • Maintain client records and documents as required by program standards and funding requirements.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Gather, edit, and refine program boilerplates and impact data. Help to keep files organized and updated.
  • Adhere to all CCC policies, procedures, and guidelines.
  • Attending all required meetings and training.
  • Perform related duties as assigned, within the scope of the role.

  • Knowledge of and ability to maintain professional boundaries
  • Experience facilitating training, groups and workshops.
  • Ability to manage time with multiple tasks in a fast-paced environment.
