SIS Assessor (Buncombe/Henderson/Transylvania County, NC)

LOCATION: Remote - will travel to and must live in or near Buncombe, Henderson, or Transylvania County. Incumbent in this role must live in North Carolina or within 40 miles

GENERAL STATEMENT OF JOB

The Supports Intensity Scale (SIS) Assessor is responsible for scheduling and administering the SIS, to include facilitating the SIS interview process, recording the scores, entering the scores into an online data base and accessing a summary report from SIS Online. This position is also responsible for interacting with American Association on Intellectual and Developmental Disabilities (AAIDD) and/or any certification process required by the state for obtaining and maintaining certification and qualifications to maintain assessment standards.

ESSENTIAL JOB FUNCTIONS

SIS Assessment/Evaluation and Collaboration:

  • Schedule and ensure appropriate administration of the SIS evaluation and that evaluations are done in a timely manner
  • Carry out SIS activities as required including uploading the scores into SIS Online in a timely manner, ensuring the report is available in the electronic health record and that a copy is given to the assigned Care Manager prior to the member's care plan meeting
  • Schedule emergent slot SIS assessments, work with the management of Registry of Unmet Needs to ensure those top members have current SIS assessment, ensure the Slot list is current, etc.
  • Interface with members, Legally Responsible Persons, providers and other stakeholders to determine satisfaction with SIS Assessment activities and to disseminate educational material and presentations
  • Ensure responsiveness to feedback/questions/concerns from individuals, families, providers and stakeholders related to the SIS Assessment
  • Interact and maintain communication with NC Division of Mental Health, Developmental Disabilities, and Substance Abuse Services; NC Division of Health Benefits and American Association on Intellectual and Developmental Disabilities in regards to Resource Allocation and SIS Implementation
  • SIS-A or SIS-C Trainer will follow all the requirements needed to obtain and/or retain their training status. As needed the Trainers will train future assessors, participate in Community of Practice meetings as determined by the state and AAIDD, and provide continuing education on as needed basis for current assessors
  • SIS-A or SIS-C Trainer they will be required to attend all required IRQR re-evaluation and Community of Practice meetings scheduled by the state. The trainers will be required to meet the criteria set by NC to keep their training status

  • Have thorough knowledge of the SIS assessment instrument, the knowledge of the SIS Online database, basic knowledge of NC Innovations Waiver and Care Management to work in conjunction with Intellectual/developmental disability Care Management staff
  • Ability to express ideas clearly/concisely
  • Ability to drive and sit for extended periods of time (including in rural areas)
  • Represent Vaya in a professional manner
  • An ability to initiate and build relationships with people in an open, friendly, and accepting manner

  • A result and success-oriented mentality, conveying a sense of urgency and driving issues to closure
  • Comfort with adapting and adjusting to multiple demands, shifting priorities, ambiguity, and rapid change
  • Proficiency Microsoft Office and Vaya systems, to include Excel, data analysis, and secondary research
  • Demonstrated knowledge of the assessment and treatment of developmental disabilities, without co-occurring mental illness
  • Have highly effective communication
  • Knowledge in Vaya Medicaid B and NC Innovations Waiver, and accreditations and apply this knowledge in problem-solving and responding to questions/inquiries
  • Have a dynamic, proactive approach to assessment, screening, monitoring and coordination of care, to ensure quality supports and consistent adherence to waiver requirements
  • This is a mobile position with work done in a variety of locations spending a considerable amount of time in the field

QUALIFICATIONS & EDUCATION REQUIREMENTS

  • A Bachelor's Degree in a human services area and two (2) years of full-time, post-degree accumulated professional experience with the population served

OR

  • A Bachelor's Degree in a field other than human services and four (4) years of full-time, post degree accumulated professional experience with the population served

OR

  • A Bachelor's Degree in nursing and licensure as an RN and four (4) years of full-time, post degree accumulated professional experience with the population served

*Must qualify as a QP for Developmental Disabilities per 10A NCAC 27G .0104

Licensure/Certification Required:

  • Successful completion of training and certification by the American Association on Intellectual and Developmental Disabilities (or the certification and training process the state mandates) within 6 months from date of hire.
  • If incumbent has a degree in Nursing, incumbent must also hold licensure as an RN

Preferred Certification: Trained and certified by American Association on Intellectual and Developmental Disabilities.

PHYSICAL REQUIREMENTS

  • Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
  • Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers.
  • Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
  • Mental concentration is required in all aspects of work.
  • Ability to drive and sit for extended periods of time (including in rural areas)

RESIDENCY REQUIREMENT: The person in this position is required to reside in North Carolina or within 40 miles of the North Carolina border.

SALARY: Depending on qualifications & experience of candidate. This position is non-exempt and is eligible for overtime compensation.
