What You'll Do

We're excited to be adding an On-Call Case Manager, Stability to our growing team! This is an engaging and rewarding opportunity that plays an integral role within the organization. We are looking for someone who is passionate about working with our participants and contributing to the fight against homelessness. The On-Call Case Manager, Stability will report directly to the Clinical Support Manager and be part of the Housing Solutions team. This individual is responsible for providing short term, focused support to families during their initial intake, housing search and/or as a provider of specific case management services to families during their participation in the rental subsidy program. This role provides administrative support to full-time staff members who are out on vacation or leave, or when there are high vacancy rates in key positions. This position is intended to be short term and to ensure that all families have access to a supportive, empathetic and resourceful staff member so their needs are addressed in a timely fashion.

What Your Day Looks Like

  • Conduct assessments, check-ins, meetings, and phone calls to provide focused, short-term support to families, ensuring continuity of care
  • Collect necessary documentation from participants, such as income verification, proof of rental payments, and progress toward goals
  • Offer supportive interventions, including goal setting, housing advocacy, and referrals for budgeting, employment, legal services, mental health, domestic violence, and substance abuse support
  • Help participants set and achieve housing goals, adjust goals as needed, and evaluate progress
  • Follow up with participants after program completion to assess outcomes
  • Always maintain professional boundaries with participants
  • Collaborate with internal and external service providers and community partners to help participants reach their goals
  • Keep accurate records and assist with audits, documentation, and record-keeping as needed
  • Ensure proper case management documentation and timely database entries for participant records
  • Work occasional evenings and weekends for program activities, as needed.
  • Perform other duties as requested

What Makes You Successful in This Role

  • Ability to work and collaborate with a diverse group of people on projects and making decisions for the benefit of our participants
  • Familiarity with case management databases or customer service platforms
  • Highly organized, self-motivated, and capable of working independently to meet goals
  • Demonstrated ability to exercise appropriate judgement and uphold policies and procedures
  • Skilled in documentation, reporting, and maintaining confidentiality

What You Bring

  • 1+ years of experience in social work, psychology, human services setting or related field
  • A background in working with nonprofit organizations, social services organizations, or community-based programs
  • Familiarity with Bay Area resources for low-income families
  • Bilingual abilities, especially in Spanish
  • Highly proficient in Microsoft Office Suite, Salesforce, and other software
  • Ability to travel locally and long distance to meet with vendors and participants
