The Patient Care Technician (PCT) for Retinal Examinations and HEDIS+ functions as an extension of the Homebase clinician team by performing retinal imaging and other ancillary testing and at times may be required to assist with clinical visits both in the clinic and home setting.

Responsibilities include:



  • Performs retinal imaging and other ancillary testing which may include an DEXA, administering a PHQ-9 or other standardized screening questionnaire(s), the collection and processing of urine or fingerstick blood specimens
  • Obtains accurate vital signs following protocols approved by Medical Director
  • Records patient data in charts in a timely and accurate manner.
  • Maintains including inventory of supplies and equipment.
  • Recognizes and responds to emergencies following protocols approved by Medical Director
  • Schedules, coordinates, and monitors appointments to support timely and effective care.
  • Communicates abnormal results of ancillary tests with the National Director of Clinical Practice
  • Updates and maintains departmental documents, as directed.
  • Functions as supply coordinator as needed, including order entry, requisition/invoice reconciliation, filing systems and monthly usage reports.
  • Performs general office administrative and support services, typing, copying, faxing, etc. Creates professional looking correspondence, completes purchase requisitions, office supply requests, and other company forms.
  • Ability to drive to patient homes or clinic setting in the assigned geography.
  • Actively support the achievement of Homebase's Vision and Goals.
  • Other duties as assigned


Qualifications and experience:



  • GED/High School or equivalent.
  • Current Basic Life Support Certification required.
  • 3+ years front and back-office experience in a professional medical office environment or field-based setting.
  • Completion of medical assistant program, required.
  • Ophthalmology Technician Cert or Certified Medical Assistant (CMA), preferred.
  • Effective, respectful, and courteous communication skills required.
  • General knowledge of office equipment.
  • Experience in MS Office.
  • Bilingual Spanish, Vietnamese, Cantonese, or Mandarin is preferred.
  • Ability to drive to patient homes and/or partner clinics in assigned geography.
  • Must have a valid CA driver's license, and clean driving record
  • Must maintain state minimum liability vehicle insurance
  • Must have reliable transportation.
  • Ability to stoop, kneel and crouch required.
  • Ability to bend, reach, lift, pull, push and/or carry up to 10 pounds regularly and up to 50 pounds occasionally required.
