Patient Navigator, Autism

• Demonstrates effective teamwork, collaboration, and communication with and among the members of the health care team, community resources and the patient and family.
• Provides assistance/support for children/families diagnosed with an Autism Spectrum Disorder, guiding them in identification/navigation of services and resources available within RB&C and in the community.
• Provides community education and outreach related to Autism Spectrum Disorders.
• Provides Advocacy for patients and families.
• Tracks interventions and outcomes of patients/families provided with Autism Navigator services.
• Provides resident and clinician education and training.
• Provides mental health counseling and supports.

• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
