Empath Health is seeking a dedicated and compassionate Registered Nurse (RN) to join the team as a Case Manager.

Schedule: Monday-Friday, 8a-5p. Area of coverage Manatee County and North Sarasota County.

Empath Health is seeking a RN Case Manager to join our team.

Primary responsibilities include:

• Conducting initial and ongoing assessments of patients and families to identify their physical, emotional and spiritual needs
• Develop and implement individualized care plans to meet the unique needs of each patient and family
• Coordinate and collaborate with the interdisciplinary team including physicians, social workers and other healthcare professionals
• Provide education and support to patients and families to promote comfort, dignity and quality of life

Educational/Professional:

1. Graduate of approved nursing program and current unrestricted Florida RN license.
2. Valid and current CPR certification required.
3. Minimum one year experience in an acute-care Hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
4. CHPN certification is strongly encouraged within three years of employment.
5. BSN or MSN preferred.
6. An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
