Description The Social Worker MSW provides services to patients/residents and their families/caregivers to meet their social and emotional needs as they relate to their medical diagnoses and concerns. They collaborate with the multidisciplinary team to ensure that patients receive treatment, recovery, and safe and coordinated transition from one care environment to another. They work to protect and advocate for the rights of patients/residents and ensures that they receive highest practicable physical, mental and psychosocial well-being of each patient. The Social Worker MSW works under the direction of Care Management Leadership and is employed by Providence Health & Services. The position will practice in accordance with competencies and guidelines outlined in Standards for Social Work Practice in Health Care Settings and the Code of Ethics as defined by the National Association of Social Workers. About the Team Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
