Summary:

Responsible for capturing data from medical records for the purposes of reimbursement, statistics, research/studies, resource allocation and benchmarking.

Responsibilities:

1.Assigns ICD-10 diagnosis and procedure codes to outpatient accounts/Applies CPT codes to outpatient accounts.

2.Assigns appropriate code(s) to diagnoses and procedures.

3.Determines appropriate sequencing of codes based on coding guidelines.

4.Appends appropriate modifiers when necessary.

5.Identifies insufficient/incomplete documentation within the medical record and solicits response from physicians.

6.Assigns appropriate discharge disposition based on physician orders and care management documentation.

7.Flags records with deficiencies and places appropriate comment in EHR.

8.Confirms release for billing in the coding perspective.

9.Serves as a resource to other hospital personnel with respect to coding.

10.Communicates with the business office to resolve coding issues related to billing.

11.Monitors edits from the business office and responds in a timely manner.

Other information:

*Associate Degree - Healthcare in Health Information Technology

*Graduate from Health Information Technology

*Registered Health Information Technician (RHIT). AHIMA approved *1 year of experience in Healthcare/Medical - Medical Coder

*Experience in ICD-9, CPT, and ICD-10 preferred

*Newly credentialed applicants are eligible to apply
