Case Manager II We are seeking a Case Manager II to work on our Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. Hours: Full-Time, Monday- Friday, business hours 8am-4pm Locations: Anoka County, Minnesota | Hybrid Hiring Range: $23.00-$26.00/Hourly based on qualifications Hiring Bonus: $1000 to external candidates People Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota.
