Description:

The Program Service Aide is a healthcare advocate responsible for performing various patient care activities, including social service coordination in community hospitals, clinics, schools, and communities. The Program Services Aide provides outreach, resources, education, and social support as outlined in specific programs.

Duties & Responsibilities:

Community Engagement
  • Host, participate in, coordinate, and sponsor community functions to raise awareness of cancer screening services and achieve project goals.
  • Provide educational information and outreach regarding programs, associated requirements, and other opportunities to community residents and the public. Assist them in making informed decisions about healthy lifestyles and healthcare needs.
  • Conduct community-based visits to identify and recruit potential new patients and community partners.
    Resource and Referral Management:
  • Serve as a resource for program participants; develop referral relationships with healthcare providers and other community-based service providers.
  • Act as a liaison between the institution or agency and the individuals served; refer patients to support services and follow up on referrals; assist participants in utilizing referral services.

Collaboration and Assessment
  • Collaborate with staff and community residents to assess community strengths, problems, and concerns related to healthcare (e.g., conducting surveys, interviews, etc.). Identify and assist in establishing needed services within the community.

Patient Assistance
  • Assist patients with the health marketplace enrollment process.
  • Enroll patients into the Illinois Breast and Cervical Cancer Program (IBCCP).
  • Schedule patient appointments via the practice management system or other indicated systems.

Administrative Duties
  • Assist in the development of resource materials and the preparation and maintenance of records and data.
  • Complete all required reports to document activities and provide data to measure impact.
  • Perform other related duties and participate in special projects and assignments as assigned.


Minimum Qualifications Required:

1. Any one or any combination totaling two (2) years (24 months) from the following categories:

A. college course work in behavioral science, social work, sociology, psychology, or a closely related field as measured by the following conversion table or its proportional equivalent:
  • 30 semester hours equals one (1) year (12 months)
  • Associate's Degree (60 semester hours) equals eighteen months (18 months)
  • 90 semester hours equals two (2) years (24 months)
  • Bachelor's Degree (120 semester hours) equals three (3) years (36 months)

B. work experience that provided a knowledge of human behavior by having demonstrated through public contact the ability to cope with a variety of people, (such as customer service, salesperson, or receptionist).

Preferred Qualifications:

  • Bilingual (English/Spanish) highly preferred.
