Responsibilities
- Serves as the first line of customer service for external and internal customers (care center relationship).
- Professionally responds through electronic referral platforms within the specified time requirements.
- Performs reverse marketing in coordination with the local BD leader to reinforce messaging, stay top of mind and present information where it may be of value to customers.
- Supports e-referral user administrative requests including new user setup and terminations.
- Reviews trends on requests and needs of referral sources. Escalates opportunities and customer concerns to manager and business development leaders.
- Meets assigned key performance indicators (KPIs).
- Receives and documents patient referrals to hospice services through electronic referral systems, ensuring compliance with applicable coverage criteria.
- Coordinates and communicates with hospice intake coordinators or designee(s) to enter referrals into homecare homebase.
- Gathers appropriate information to assist the clinical team in scheduling an evaluation visit.
- Provides the appropriate information from the referral system to the hospice intake coordinator to ensure complete referral information.
- Performs other duties as assigned.
Qualifications
Requirements:
- High School diploma or GED equivalent.
- Experience working in an environment where timeliness and quality of response were measured and impacted business results.
- Six months experience in customer-facing in sales or operations.
The schedule for this position varies with rotating weekends, evenings and holidays.
