THE POSITION

DESCRIPTION OF WORK

As an Aging Care Manager 2, you will provide case management services to an assigned caseload of consumers. Upon successful completion of protective service training, you will be responsible for conducting thorough investigations of assigned reports of older adults who need protection from abuse, exploitation, abandonment, or self-neglect. You will have the opportunity to establish effective working relationships with older adults, family members, friends, neighbors, social workers, pastors, attorneys, medical personnel, or other collaborative partners in order to provide various care management and protective services for adults in your community. You will also be responsible for maintaining up-to-date documentation in the database as well as coordinating additional services

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • On-call hours and overtime is required.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Successful completion of the County Social Casework Intern program; or
  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Potter County. If no eligible candidates who live within Potter County apply for this position, candidates who reside in Cameron, Clinton, Elk, Lycoming, McKean, or Tioga counties will be considered. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

Legal Requirements:

  • You must pass a background investigation.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    1. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
