Social Worker- Berryhill Clinic UnityPoint Health - Fort Dodge FTE: 1.0 (Full-time, 40 hours per week) Social work role works in various areas of Berryhill to assist patients and families in understanding patient conditions and treatments. They can help patients and family members develop coping strategies, assess patient needs, assist in the development of crisis plans and providing emotional and mental assessments on patients that present to the clinic for behavioral health concerns. This role will make, monitor, and track new patients and referrals to assist with care coordination and care establishment with our CCBHC, DOC and all program and services. Why UnityPoint Health?
Visit us at UnityPoint.org/careers to hear more from our team members about why UnityPoint Health is a great place to work. https://dayinthelife.unitypoint.org/ Responsibilities * Evaluates the need for and provides psychosocial counseling to patients and families. * Assess patient/family social and emotional factors to estimate their capacity and potential to cope with illness and as appropriate, death. * Assist to provide medically related social services to assist patients in maintaining or improving their ability to manage their everyday physical, mental, and psychosocial needs. * Assists to assess post-hospital needs. Refers patients and families/significant other to appropriate resources. * Responds to crisis related walk in's or calls. * Participates in interdisciplinary team meetings. * Maintain timely and accurate documentation in patient's medical record on identified patients. * Assess social, emotional, and economic needs in relation to the impact of behavioral health illness. * Works closely with financial advocates to ensure that patients in financial need are connected to appropriate resources. * Provides immediate intervention including supportive counseling to patient/family in dealing with impact of an emergency. * Connects with DCO's to collect the required DCO patient data and metrics per CCBHC criteria. * Works within the guidelines of the grant and coordinates activities associated with grant-funded programs. * Maintain positive working relationships with outside entities and Community Network Providers. * Work with Network Providers to establish and improve community relationship to improve population health * Adheres to the Social Work Code of Ethics. * Maintains effective communication/working relationships with co-workers, management and other hospital staff. Behave in manner consistent with Standards of Behavior. * Participates in educational activities, performance improvement activities, hospital committees, and other related activities. Qualifications Education: Baccalaureate Degree in Social Work from a school accredited by the Council on Social Work Education. May accept degree in Psychology, Sociology or other field related to social work depending on candidate. Experience: One-year previous social work experience License(s)/Certification(s): Valid driver's license when driving any vehicle for work-related reasons. Knowledge/Skills/Abilities: Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to adult and geriatric patients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills.
