Job Summary :

Under the general supervision and work direction of the staff pharmacist, the Pharmacy Technician for Medication Reconciliation performs pharmacy-related functions, in compliance with departmental policies and procedures that provide optimal pharmaceutical care.

Job Description

Essential Responsibilities:

1. Develops an accurate list of current "home" medications using all available resources.
a. Conducts patient and/or caregiver interviews
b. Contacts community pharmacies, mail-order pharmacies or other sources of medications.
c. Utilizes other computer resources available
2. Discusses the list with a pharmacist for approval.
3. Enters the list into the hospital computer system according to established procedures.
4. Assists other health care professionals with Medication Reconciliation issues.
5. Provides general assistance to pharmacist staff.
a. Assists pharmacist in order entry area as needed.
b. Accurately pulls medications for checking.
c. Labels medication accurately and appropriately when not available in unit of use packaging.
6. Performs miscellaneous duties and follows guidelines and policies/procedures.
a. Uses appropriate judgment; refers problems to a pharmacist as needed.
b. Organizes time effectively to meet changing workload demands.
c. Demonstrates a consistent level of performance.

Education and Experience Required:

1. High school diploma
2. Three (3) years experience in hospital and/or community pharmacy highly desirable, but not required.

Mandatory Licensure/Certifications:

1. Current valid State of Indiana Board of Pharmacy license as Pharmacy Technician or Pharmacy Technician-in-training
2. Current National Certified Technician program completion or completion within one (1) year of employment highly preferred
