RN-Nocturnal Team Lead

Responsibilities

  • Assists all patients and care team members throughout the care model by acting as a patient advocate.
  • Connect with providers and patients regularly via a telehealth platform and receive regular updates on patient activity.
  • Serves as a subject matter expert across all care coordination.
  • Ensures that all new CCs and CPs have the appropriate tools and templates to operate.
  • Serves as a SME for triage and management of standing orders in patient care across the continuum.
  • Supports VCU manager in coordination of clinical and operational workflows across markets and documents.
  • Communicates discharge information to other clinical departments or members of the care team.
  • Builds and maintains collaborative professional working relationships with all care team members and is available to provide guidance and support during times of urgency or critical needs.
  • Monitors the care that the patient receives and provides real time coaching and mentoring.
  • Assists as needed in recruiting, interviewing, and training of new team members.
  • Performs other duties as assigned.

Qualifications

  • Associate of Science in Nursing
  • Current, active RN licensure in good standing and willing to be licensed in multiple states at Contessa's expense.
  • Three to five years of chronic care coordination and independent triage support. Possess a general knowledge of clinical standards and outcome measurement.
  • Must have reliable transportation and proof of liability insurance.
  • Ability to support after-hours schedules and on-call needs.

Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.
