Job Description

As part of a case management team, provides transition services for incarcerated offenders; provides record keeping, instructional assistance, and interaction and communication with clients, staff, parole agents, and other agencies.

  • Identifies eligible candidates, facilitate program orientations, complete evidence-based risk/needs assessment
  • Maintains a caseload, builds transition plans for clients to include individual case planning, composes and collets required documents
  • Assists participants with completing various forms and obtaining documents for transition, training, education, and employment
  • Reviews documents for accuracy, completeness, and compliance
  • Compiles data for reports as needed
  • Assists participants in gaining and retaining employment and provides referrals as necessary
  • Networks with community providers, employers, and partners to establish vital contacts for case planning, training, education, and employment
  • Tracks and evaluates participant's program compliance and non-compliance by working with agency divisions, partners, vendors, and re-entry team
  • Resolves participant requests regarding payment issues, free-time, special purpose passes, and grievances
  • Maintains and adheres to site policies, procedures, and regulations and ensures confidentiality of all data/information
  • Other duties as assigned
Qualifications
  • High School Diploma or equivalent with two years of related experience preferred
  • Excellent computer skills and knowledge of Microsoft Office, Word, Excel, Power Point
  • Skills in communicating with diverse populations
  • Experience performing case management and transition plans preferred
  • Ability to work independently and have well developed interpersonal skills
  • Experience in criminal justice or related human service field preferred
  • Ability to travel as needed at least 25%
