JOB SUMMARY
Organizes and conducts medically prescribed therapy programs in the home/daycare center to restore function to the patient.

- Licenses and Certifications
(BLS) BASIC LIFE SUPPORT or Cardio-Pulmonary Resuscitation certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire
(DRIVERS) DRIVERS LICENSE Valid Texas driver's license, proof of driver's insurance Upon Hire
(LPT) LIC PHYSICAL THERAPIST current licensure as Physical Therapist is required Upon Hire

JOB DUTIES
- Directs and aids patient in active and passive exercises, muscle re-education, and functional training using pulleys, weights, steps, and inclined surfaces.
- Provides instructions in therapeutic procedures to be continued by the patient/family.
- Adapts conventional physical therapy techniques to meet the needs of patients unable to comprehend verbal commands or voluntarily carry out a regime of therapeutic exercises.
- Designs physical therapy programs and sets goals for each patient.
- Evaluates, records, and reports on patient's progress for review by members of the care team.
- Maintains patient records and charts to ensure an accurate record of treatment.
- Responsible for monitoring delivery of physical therapy services in adherence to agency's clinical standards.
- Participates in Continuous Quality Improvement (CQI) Program.
- Provides supervisory visits for Physical Therapy Assistant as required by state and federal regulations.
- Responsible for participating in on-call rotation and emergency call according to agency policy.
- Performs other related duties as assigned or requested.
