What You Will Do to Change Lives
The Admissions Coordinator assesses the needs of the members served and coordinates their admission to the program . The Admissions Coordinator serves as a primary support to screen clients for services and admission to the program. This includes coordinating and collaborating with team members and outside agencies such as outpatient behavioral health providers, medical providers, housing providers and other community supports.
Shifts Available:
Full Time; 8:00 am - 4:30 pm; Monday - Friday
Expected starting wage range is $21.00 - $24.11. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

QUALIFICATIONS

Outpatient Programs



  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience or Bachelor's degree in Social Services and 6 months of experience
  • Must have CA Driver's License and personal car insurance with your name listed as a driver
  • Must have personal car that you are willing to use for work
  • Must be willing to transport clients in your personal car


ESSENTIAL FUNCTIONS

* Demonstrates the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders

* Assumes responsibility for admission coordination, including:

o Receiving and screening all referrals and determining appropriateness for admission, with final approval by the Program Director or authorized designee

o Developing and maintaining a professional relationship with the County contract monitor, specialized services, and all referral sources

* Gathers collateral information following admission to assess social service and treatment needs, including history and current situation. This can include meeting with the member served, family, and team members.

* May coordinate admissions paperwork and support other duties such as tours, scheduling meetings, etc.

Outpatient or Community Programs

o Actively participates in team meetings and service planning meetings as requested

o Work effectively as part of a multidisciplinary team

o Provide appropriate interventions in crisis situations
