If you are reside in the state of Massachusetts, you will have the flexibility to work remotely* as you take on some tough challenges.

Responsible for supporting patient and providers in ensuring high quality care and safety by clinical follow up according to standard criteria of patients that are identified in the analytic registries.

Primary Responsibilities:



  • Provides concurrent and retrospective abstraction accurately for the Registry patients
  • Reviews records and related detailed data in area of registry focus
  • Determines the appropriate action to be taken involving decision making according the standards criteria
  • Review new patients on the list on a regular basis using the Registry list for patient outreach
  • Communicates with providers and patients systematically via EMR or telephonically
  • Maintains tracking of patient lists, outreach and follow up
  • Offers clinician and patient support and/or education on workflows and protocols in the identified registry
  • Works with assigned supervisor/manager and director to develop, implement, and monitor procedures that support organizational goals and business objectives related to the Registry program
  • Ad hoc visits to clinical sites may be scheduled to enhance coordination of team-based care and facilitate clinician education
  • Complies with health and safety requirements and with regulatory agencies such as DPH, etc.
  • Completes all organizational and department specific clinical educational competencies
  • Complies with established departmental policies, procedures, objectives and guidelines
  • Adhere to the organization's policy on Confidentiality and HIPAA regulations, which protects the privacy of patient information and records. Responsible for protection of information and documentation related to the medical record review and abstraction process, which prohibits the release of any identifiable medical information or inappropriate disclosure of such information
  • Enhances professional growth and development through educational programs, seminars, etc.
  • Attends a variety of meetings, conferences, and seminars as required or directed
  • Performs other similar and related duties as required or directed
  • Regular, reliable and predicable attendance is required

Required Qualifications:



  • Graduate of an accredited School of Nursing
  • Currently licensed as an LPN in the State of Massachusetts
  • Proven knowledge of various software applications such as Microsoft Word, Excel and Outlook, etc.



Preferred Qualifications:



  • 1+ years of LPN experience
  • BLS/CPR certification or ability to obtain within 90 days from hire
  • Phone triage experience
  • Experience with EMR systems (EPIC)
