This is a hybrid position requiring both on-site and remote work. You will be working with a team to assist in connecting patients with follow-up care, complete chart reviews and follow-up based on a well-established protocol system. The team works daytime hours, typically 12-hours with occasional weekends (approx once every 8-10 weeks). This position is a collaborative role working closely with ED providers and staff. Must have good communication skills and highly motivated as this is an autonomous position. Education: Graduate of an accredited School of Nursing required. Bachelor's degree Preferred. Experience: Minimum of 3 years RN experience required. At least 2 years experience in relevant specialty or department, as defined by the department, desired (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery).
Valid Virginia, or state specific, drivers license required for employees working in Home Health or departments where the work requires traveling. About Carilion
